All qualified small businesses are required to complete the following three forms prior to their meetings with the Pennsylvania Authorized Trade Representatives. 

You must bring these completed forms with you to your meetings, they will be collected at the registration desk.

1. Self Representation as an Eligible Small Business - Form by the U.S. Small Business Administration (If you are unsure whether or not you should sign this, please click here  and follow steps 1 & 2 on the following web page)
2. Strategic Plan Outline - (To answer questions 1 and 2 please list the countries you are meeting with)
3. Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion (Form 1624)

Pennsylvania International Week is being funded by a grant from the U.S. Small Business Administration (SBA). Under the terms of the grant, the SBA requires us to obtain the above forms before your meetings. Please contact your trade specialist if you have any questions regarding these forms.
